What is Closed Rhinoplasty in Turkey?

The medical term rhinoplasty, built as a combination of the words rino (nose) and plasty (to shape or to form), means “shaping the nose” or “nose aesthetics”.

In aesthetic nose surgeries, called as rhinoplasty in general, two different techniques are used: “Open” and “Closed”. In open rhinoplasty, a “Λ” shaped cut is applied to the nasal tip, in the area between nostrils. In closed rhinoplasty, the operation is performed by entering through the nostrils and cuts are made entirely inside the nostrils, without cutting the nasal tip. An open rhinoplasty is a lot easier in terms of surgical intervention, thanks to a clear view it provided to surgeon; however, a post-op scar may be visible after the operation. Though closed rhinoplasty is a challenging technique, no scars will be visible on the nose; because all incisions are made inside the nostrils, it protects the veins, nerves, and ligaments.

Photographs of the patient are taken before rhinoplasty and the rhinoplasty is planned by using 2D simulations, in line with patient requests and surgical needs. Rhinoplasty is performed within a period of 3-4 hours depending on the physiological problem and the changes planned. The surgery is operated under general anesthesia. After the intervention, patient is discharged on the same day or the following day based on patient’s condition.

For whom nose aesthetics is appropriate?

Rhinoplasty is a solution for individuals who are not satisfied with their nose shape, appearance or size; it is also a preferable surgical technique to improve nasal functions of those having injuries and deformations or to provide healthy solutions for those experiencing respiratory problems. The general objective of this intervention is to improve the function and appearance of nose, considering the face structure along with skin type and tissue. The most common requests for rhinoplasty are to treat breathing difficulties and related sleep disorders, to reduce the size of nose, to shorten nose, to correct the size and shape of nostrils, to correct nasal bridge, to correct nose’s asymmetrical appearance, and to reshape asymmetrical nasal tip.

Rhinoplasty surgeries performed upon aesthetic demands are mostly based on personal choices.  While some people are satisfied with their appearances regardless of having a nose which is disproportionate or often considered ugly, there are also people having a nose which seems normal but wishing to have a completely different appearance. As a result, rhinoplasty surgeries are operations widely performed because of physiological health problems or personal preferences.

Is there an age limit for nose aesthetics?

Continuing growth and accompanying facial growth in young adults and teens whose physiological development is not yet completed, result in change of appearance and functionality of nose. Since rhinoplasty surgeries are performed mostly under general anesthesia, having this operation at a very young age causes stress for both children and their family. Except for the cases deemed necessary by surgeon or urgent cases, rhinoplasty surgeries are generally performed on girls older than 15-16 years and boys older than 17-18 years.

How long it takes to recover after a closed rhinoplasty?

Rhinoplasty surgery is not a painful surgery. Post-surgical pains are reduced to minimum especially thanks to improvements and developments in surgery instruments in the recent years. Yet, pain threshold varies by person depending on psychological and physiological condition of each patient. In order to reduce pain, patients should rest by holding their head half-high for a couple of days and move without leaning as much as possible.

The need for tampon after surgery become less day by day thanks to advanced technology and medical equipment. Silicone splints with air holes allowing the patient to breathe are widely used. 

During operation, blood penetrating from soft tissues inside and around the bone intervened may accumulate in orbits. Thus, post-surgical bruising and swelling may occur on upper and lower eyelids. Bruising and swelling do not mean that patient has pain. With the usage of modern surgical instruments (piezosurgery, drill, motor), bruising and swelling are reduced to a minimum level and disappear in about a week. Another reason of bruising is that patient has a skin prone to bruise. A more severe post-surgical bruising may occur especially in light-skinned persons.

Silicone splints are removed two or three days after discharge on the same day or the following day. After the removal of silicone splints, patients may take a shower by washing their body without wetting their head. Splints placed inside the nose are removed in a week and nasal bands are applied. In this period, patients may take shower as normal. Nasal bands are removed on the tenth day. In closed rhinoplasty, stitches inside the nose will disappear on their own within about 15-20 days. Patient may go swimming in a pool or sea 15 days after surgery. Then, the control period by surgeon begins.

There is no limitation for eating or drinking after two days. Patients may return to their daily life upon the removal of nasal bands and splints on the tenth day. Patients should not be exposed to sun unless the bruises on their upper and lower eyelids disappear. If it is a must, patients should wear a hat. However, patients should not wear glasses for at least 2 months. This period may continue up to 6 months.
